Life and Accidental Death Coverage
•Up to eight times annual salary payout for accidental death.
•Four times annual salary payout up to $500,000 in coverage.
•No "phase in" period.
•No decline in benefits by age until 65.
•Spouse and dependent coverage included.
Long Term Disability
•Sixty percent of salary up to $5,000 per month.
•Housing allowance included.
•Lifetime security benefit is included to extend LTD benefits beyond benefit period.
•$350 per month in dependent care expenses.

Member Benefits
Church Benefits Board (CBB) members, who are enrolled in the Life and Disability Insurance Program have 10 days of International Travel Medical Coverage free of charge, it is part of your core member benefits with CBB.
How to apply: At least 3 Business days prior to your departure, please go through this quick application process:
1) Fill out the CBB Travel Certificate.
2) Fax it to CBB at 770-220-1661 for processing.
3) You will receive your Med ID & Policy at the e-mail address provided in your application.
We also provide additional days of coverage at a discounted rate for you and immediate family members. Please complete the Additional Days of Coverage Form. |
The Need

How It Works
While traveling, if you have any new sickness or injury, seek medical help immediately at any provider of your choosing. Upon seeking medical attention, contact the international number 01-317-655-4500 listed on your policy card as well as on your claim form. You will be guided through the process of what to do for the country in which you are traveling. This number is the same for any country you may be located. Please see your policy for full details upon enrolling.
